My money is on the 13161/13078 interface, on this diagram:

I've seen it happen, on Honda XR.
Honda XR have, not exactly behind the clutch basket (a little bit
forward it, same position for all XRs I tinkered with) a thingy like a
disc that by the power of a spring make pressure on a star shaped part
attached to the the 'other end' of the shift lever to make it return
to the center position. I've witnessed a friend crash really hard to
the left, pushing the shifter lever and the star shaped part in,
towards the engine, and that caused the disc not 'fit' the star shaped
thingy properly, giving the exact same symptoms Rob is mentioning.
Seems Kaws use a different system, but it's similar and could perhaps
cause the problem. I'd check that first.
To bend a shift fork one needs to really beat on the gearbox. Not sure
on Kaws, but on Hondas and Yamahas it's very hard to be so brute as to
bend a shift fork. Worn out dogs on a 2007? Well, it might happen, but
I'd not expect that for the next 10 years...

If you look at the transmission:

3-4 5 gears are selected by moving 13262, gear input 3rd
side to side on the input shaft. That selects 3rd, 4th and 5th
by engaging dogs to the 4th and 5th input gears. I'd bet
the fork that moves it and the dogs on it and 4th and 5th
are screwed.
Go fast. Take chances.
